Chicago Dermatological Society CME
The CDS offers its members continuing medical education credit through joint sponsorship with organizations accredtited through the ACCME. Stringent standards and procedures are needed to secure Category 1 accreditation. Among these are a needs analysis, identification of gaps in knowledge to be addressed by the educational activity, and learning objectives for each conference.
Prior to February 2010, the CDS worked with the Chicago Medical Society (CMS) for joint sponsorship of CME. Credits earned through the Chicago Medical Society are documented in a certificate mailed to the physician by the CMS. If you need to retrieve documentation for meetings accredited by the Chicago Medical Society, please contact them directly at: (312) 670-2550.
For meetings taking place in 2010 through June 2012, our current joint sponsor -- the Colorado Foundation for Medical Care -- used an online meeting evaluation and CME claim process which, when you complete the short form, will result in your CME certificate being sent to you by email instantly. Beginning in October 2012, certificates for credits earned will be send automatically to attendees who sign the registration sheets at the meeting.
Information about the claims process
- All physicians claiming continuing medical education credit as a result of attending a CDS conference must complete the online form provided through our joint sponsor. CME certificates are not sent automatically.
- The sign-in sheet at the meeting is our proof that you were present, but it is not the actual CME claim.
- To complete the online claim, go to the website (links below) and nter your name, your email address and answer all of the questions on the short online evaluation form.
- Upon completion, click on the "submit" button.
- An email will be sent to you almost immediately with a PDF file attachment. This is your certificate showing the total number of CME credits available at the conference. NO OTHER DOCUMENTATION WILL BE SENT TO YOU.
- It is your responsibility to save the documentation and to record the number of credits you actually earned. Neither the Chicago Dermatological Society nor our joint provider (Colorado Foundation for Medical Care) track the CME credits you receive.
Here are links to the Colorado Foundation for Medical Care online claim forms. You should complete your online within 60 days following a meeting. However, these links usually are active for much longer.
